Local jargon This was obviously written to be read by people who know what "fazer cola" means. In an ideal world, you'd know where this report came from and would be able to just call someone from there and ask for clarification. Healthcare professionals are usually quite solicit in quick technical matters.

In Brazil, "fazer uma cola" means to write a reminder, a small note, for something. Very common in election days, when some voters bring a "cola" with their candidates' numbers. I know this is PT-PT, but maybe the doctor is Brazilian? It's a possibility, that the patient wrote his symptoms/issues on a paper in order to be able to remember and tell them at the ER.

Portuñol? Perhaps an influence from Spanish "cola", meaning the patient had to stand in line. The best course of action in this case is to clarify the meaning with your client. Portuguese "cola" seems to make no sense in this context.
