Although there are no direct references to translations for the whole term and I do not have medical science background, I think your word-by-word translation is straightforward and is understood by people in medical field.

I found that a revised version of "International Statistical Classification of Diseases and Related Health Problems", which is called "ICD-10", has an entry of "G45.1: carotid artery syndrome (hemispheric)". It is translated as "G45.1 頸動脈症候群(半球性)" in Ministry of Health, Labour and Welfare's Web site.

I also found that "高血圧性***" translated as "hypertensive ***" in the following reference.

So, I would choose to use "hypertensive carotid artery syndrome".
