Hi all,
This is from a Spanish cardiology discharge report. The patient's history starts with:
"GBF por disnea II-III/IV"
And also includes, high BP, type II DM, and then the cardiological history.
I can't figure out or find anything to help me with "GBF". Any clues? TIA

El único "salto" aquí es la referencia explícita a la clasificación de la NYHA, sólo implícita en el original -aunque creo que suficientemente justificada, tanto por generalidad de uso de la escala NYHA en este contexto como por las pocas referencias al acrónimo original, que resultan concordantes, o al uso explícito de la numeración romana sobre un máximo definido por el grado/clase IV, también concordante.

As for translation, I'd probably use NYHA class II-III (heart failure) (or at least start from there my revision of the original text to see if I can reject the hypothesis).

This reference says that the "(NYHA) classification is used to grade the severity of functional limitations in a patient with heart failure:" This classification system has 4 classes: I, II, III & IV, which corresponds to the text of the question.
Having said all that, I still haven't been able to track down anything on "GBH", so I'm not certain that the NYHA system is what's going on here.
Further, Asker states that the test "is from a Spanish cardiology discharge report." So I would expect that the grades listed, " II-III/IV" are related to some kind of cardiac health evaluation scale; & furthermore, one that utilizes Roman numerals.
