GBF

Spanish to English Medical Medical (general)
Hi all,

This is from a Spanish cardiology discharge report. The patient's history starts with:

"GBF por disnea II-III/IV"

And also includes, high BP, type II DM, and then the cardiological history.
I can't figure out or find anything to help me with "GBF". Any clues? TIA

La insuficiencia cardiaca (heart failure) puede producir distintos grados de disnea en función de la actividad física, desde una disnea de reposo o una disnea de mínimos esfuerzos (por ejemplo levantarse del sillón o incluso hablar), a una disnea de esfuerzo (pej. al subir escaleras) o sólo de grandes esfuerzos (pej. elevar grandes pesos). Las clasificaciones funcionales de la insuficiencia cardíaca evalúan el grado de limitación que sufre un sujeto en base al tipo de actividad física que logra producir disnea. Referirnos así a una clasificación funcional en base a la disnea (GBF por disnea) o a una clasificación funcional de la insuficiencia cardíaca sería equivalente en este caso.

El único "salto" aquí es la referencia explícita a la clasificación de la NYHA, sólo implícita en el original -aunque creo que suficientemente justificada, tanto por generalidad de uso de la escala NYHA en este contexto como por las pocas referencias al acrónimo original, que resultan concordantes, o al uso explícito de la numeración romana sobre un máximo definido por el grado/clase IV, también concordante.

This heart failure symptoms assessment evaluates the level of physical activity that induces dyspnea. Even though they might sound different, NYHA class and grado [clase] funcional por disnea refer in this case to the same type of clinical limitations.

Supposing it is "NYHA II-III/IV heart failure" - what about the "por disnea", it seems to be ignored in this solution?

The numerals do look like the NHYA now I look again, and it's a cardiology discharge report, so... Marie's 'Grado basal funcional' seems right, but for the NYHA.

The original seems to refer to NYHA functional classification (based on heart failure symptoms).

As for translation, I'd probably use NYHA class II-III (heart failure) (or at least start from there my revision of the original text to see if I can reject the hypothesis).

(Continued) I'm guessing that the text refers to the New York Heart Association (NYHA) classification for chronic heart failure (CHF). This is explained in this document:

This reference says that the "(NYHA) classification is used to grade the severity of functional limitations in a patient with heart failure:" This classification system has 4 classes: I, II, III & IV, which corresponds to the text of the question.
Having said all that, I still haven't been able to track down anything on "GBH", so I'm not certain that the NYHA system is what's going on here.

@Marie I don't think we are talking about the Mahler Baseline Dyspnea Index here. First, the BDI does not use Roman numerals - it uses Arabic numerals. The range also seems to be off - the BDI is the sum of responses in 3 categories, each of which can range from 0 to 4; so the BDI summed up can range from 0 to 12.
Further, Asker states that the test "is from a Spanish cardiology discharge report." So I would expect that the grades listed, " II-III/IV" are related to some kind of cardiac health evaluation scale; & furthermore, one that utilizes Roman numerals.
