ΠΡΑΞΗ ΕΜΦΑΝΙΣΗΣ ΚΑΙ ΔΗΛΩΣΗ
Στην Αττική σήμερα, στις 10 του μηνός ....παρουσιάστηκε σε εμένα τη Συμβολαιογράφο τάδε, η μη εξαιρουμένη από το νόμο: ΧΧΥ, *με τάδε στοιχεία* η οποία ζήτησε τη σύνταξη και υπογραφή της παρούσας πράξης, με την οποία δήλωσε υπεύθυνα ενώπιόν μου ότι: "είμαι η ΧΧΥ, *με τάδε στοιχεία*".

Sorry, transphy, I misunderstood your answer. Thank you for the detailed explanation.
I cannot give the text because it contains personal data. I just gave it as an explanation of what the document contains for better understanding. The phrase which I asked about is the title of the document.

My suggestion is not of what they call it in Spain or in Goa but how English Legalese will fit in the sentence after '....ο ενδιαφερόμενος λέει "είμαι ο τάδε με τα τάδε στοιχεία", τα οποία βεβαιώνει ο συμβολαιογράφος.' In my example it will be said that....the interested party will say, I am XXX XXXX, (name) of XXXX(address) and this action/deed took place in my presence AND this is what I state......... Simply put!!!

In the above you say 'hereby state' instead of 'hereby certify'. There is NO Notarial Services in the UK and the word 'Declare' is often replaced by 'hereby state' and 'εμφάνισης' by 'in my presence' when you are drafting Documents at a Commissioner of Oath' (Notarial equivalent).

In other cases, there is, “notice of appearance and declaration". See:

but I do not think that this is suitable in your case.

In the circumstances, I would probably translate this as, "deed of appearance and declaration".

I do not know if an exact alternative exists in English notarial practice, but the following seem to fit at least for “πράξη δήλωσης” (notarial deed of declaration and deed of declaration):

Perhaps, the English take it for granted that anyone making a declaration must have “appeared” in some way in order to be able to make a declaration, and therefore see no reason to confirm the fact.

This is the title of the document. It says that before the notary the interested party, whose details are bla bla, appeared and stated that: "My name is X and my details are bla bla". That is all.

What type of document are these words included in? Is this included in a property transfer or sale deed, or what? I.e., what type of document is the notary compiling?
