This is in the subject line of a memo regarding the transfer of an Ethics Committee from one clinical trial study site to another. The full line is
Ref: Alcance al radicado No. [xxx] de fecha de [xxx] Protocolo aprobado en acta xx de [año]. [Colombian Spanish, by the way, to be translated to American English]
What I have so far, and it doesn't entirely convince me, is "Scope of file No. [xxx] of [date] Protocol approved in minutes #[xx] of [year]." [note that "radicado" refers to registered filings in Colombia; nothing to do with location of a person or place].
The memo then goes into what I explained above.
Any suggestions appreciated.

What caught my attention here, and makes you wonder, is that it's "alcance al", not "alcance del".
As well as the more familiar meanings of "alcance", a number of examples from Colombia and Ecuador clearly suggest that it can mean a document or documents presented as a supplement to an existing file. I can't find a relevant definition in any of the dictionaries available to me, and I'm not sure if the precise sense is attachment or perhaps amendment or correction, but "addition" seems to be the general idea. See these examples:
