Change of heart is a sudden change in decision, in the course of one's life, but it is not an idea or something logical. It is a *heartfelt* thing, one that you cannot resist.
Think Kate Winslet jumping back to the Titanic when she was already in the lifeboat.
Think Darth Vader suddenly grabbing the emperor and casting him down the shaft instead of finishing off Luke Skywalker...
So it *is* a change in the heart, that you can actually feel in the chest, and in this line the word "corazonada" is the strongest.
Usually it is about a hunch - the ominous feeling something is about to happen, "corazonada" also refers to the ominous feeling you are about to change the course of your life, and then you do it a second later.
On the other hand, this is a poem and maybe there is a play on words, the author suggesting that at some point he might change his whole heart and replace it with another, such as the bleeding heart of the aforementioned Jesus for instance...
