Apparently, from what you say, this is not an official document (such as a marriage cetificate), but rather just any old book that is owned jointly by a husband-and-wife team. So I'd just use something like the above.

Unless....

A thought just occured to me: Maybe the country where this document came from issues "libretas de familia" rather than "certificados de matrimonio"? In which case, ignore my comments above.
