I do not find any sentences using this phrase. What does exactly it signify? How do I put it in a sentence? Thank you very much dear Arabic translators.

It is not common to use them as one word in standard Arabic. It is mostly colloquial usage. Possibly to imitate قلّ ما, قلّما. But I haven't seen كثر ما or كثرما except in dialectal Arabic. The correct form is كثيراً ما, that is, 'many a time'.

The structure is colloquial, used in spoken Arabic. In fluent written form, you could say
لارتكابك هذه الأخطاء مراراً
لتكرار أخطائك المرّة تلو الأخرى
لكُثرِ أخطائك المتكرّرة
كثيراً ما ارتكبت تلك الأخطاء
كثُرَت أخطاؤك هذه
