certificate of incorporation on change of name
Attestation d'enregistrement d'un changement de dénomination sociale

Explanation:
La notion de dénomination sociale me semble plus précise que le terme "nom"

disagree  Daryo: It's not a "certificate of change of company's name" // The reason for issuing it is the change of name, but what is being "certified" is the continued existence of the company No. 4276478 - that the company bearing this new name exists at all.

Certificate of Incorporation on change of name
Attestation d'enregistrement suite au changement de dénomination sociale

Explanation:
this document is a "Certificate of Incorporation", used as proof that a company exists at all, is duly registered.

The part "on change of name" is only an additional information, it doesn't turn the whole document into a "Certificate of change of name".

To make a parallel with natural persons, a new passport issued after someone decided to change their name IS NOT the same thing as the deed pool proving the change of name.
