Creo que los equivalentes que da Lorena están muy bien. Ahora, no me imagino el sig. diálogo: -¿Cómo se siente? -No bien en lo más mínimo. Pero como sí me imagino al paciente diciendo, -Para nada bien, aún considero las opciones de Lorena, bastante bien. Por otro lado, me preguntaba por qué las demás respuestas usan bueno en lugar de bien. Pero ahora entiendo más o menos bien: supongo que piensan en la pregunta "¿Cómo considera su estado de salud?"

I interpret in hospital often and have to deal with these questions all the time, they are usually about how the patient feels, here is my version
Very good = muy bien
Rather good = bastante bien
Fairly good = mas o menos bien
not very good = no muy bien
not at all good = para nada bien, no bien en lo mas minimo

Are those terms presented in that order in the source text, as a supposedly increasing scale of 'wellbeing'? I ask the question because I would normally expect to find 'rather good' between 'fairly good and 'very good', not below 'fairly good' as in the question as posted.
