Testo originale - Inglese OBJECTIVES: To evaluate the impact of surgical treatment of stress urinary incontinence on the sexual function of women and to identify whether such treatment can improve their sexual function and overall quality of life.
MATERIALS AND METHODS: 64 heterosexual women with such indication were studied using the Female Sexual Function Index (FSFI) questionnaire, modified by introducing one question to evaluate the impact of urine loss. This was applied preoperatively and six months after surgery.
RESULT: Among these 64 patients, 60.94% had regular sexual activity, while 39.06% did not. Among sexually active patients, 59% had urine loss during sexual intercourse and, of these, 87% had urine losses in half or more of sexual relations. There were no statistically significant differences in assessments of desire, arousal, lubrication, orgasm, satisfaction and pain, or in totaling the scores, between the preoperative period and six months after surgical treatment. However, the scores for urine losses during sexual intercourse were significantly better after the operation.
CONCLUSIONS: Analysis of the results allowed the following conclusions to be reached: Urine lost during sexual activity was frequent among patients with stress urinary incontinence. Suburethral support surgery did not jeopardize sexual activity. Patients cured of stress urinary incontinence did not present improvement in sexual function.

INTRODUCTION
Epidemiological studies have suggested that the same pathological processes and risk factors that are associated to male erectile dysfunction (age, hypertension, smoking, hypercholesterolemia and previous pelvic surgery) are also associated to female sexual dysfunction (FSD) (1).
Sexual dysfunction (SD) is highly prevalent in both sexes, ranging from 10% to 52% among males and from 25% to 63% among females. Data from the Massachusetts Male Aging Study (MMAS) has shown that 34.8% of men aged 40 to 70 years have moderate SD, which is strongly related to age, associated diseases (diabetes, hypertension and hypercholesterolemia) and emotional state (1). Less is known about the epidemiological aspects of FSD (2).
Female urinary incontinence (UI) is highly prevalent. Diokno (1995) found prevalence ranging from 10% to 25% in a population within the age group from 15 to 64 years old. The numbers may increase if older populations are studied. It is worth emphasizing that, in the United States, dealing with UI has an annual cost of 10 billion dollars (3).
Although SD and UI both have high incidence, few studies have sought to correlate them aiming at assessing the impact of UI on women's sexual life. Likewise, there has been little study of the improvements that might be obtained, if any, after treatment for UI.
Considering the high prevalence of SUI in our environment, our objectives in the present study were to evaluate: The impact of UI on the sexual life of women with an indication for surgical treatment using suburethral support techniques (SST) and Whether the correction of this dysfunction might favor these patients' sexual lives, thereby causing an improvement in their quality of life.
MATERIALS AND METHODS
Sixty-four heterosexual women from the urogynecology outpatient clinic were sequentially studied between August 2001 and September 2002. All of them were indicated for treatment of SUI by means of SST. The patients' ages ranged from 33 to 86 years, with a mean of 56 years, standard deviation of 12.3 years and median of 55 years.
We chose to subjectively examine the sexual function of all the patients studied, by means of applying a modified version of the Female Sexual Function Index (FSFI) questionnaire, which was devised by UMDNJ Robert Wood Johnson Medical School, Piscataway, NJ; University of Tennessee, Memphis, TN; University of Washington, Seattle, WA; University of Texas, Austin, TX; and Columbia University School of Medicine, New York, NY (4). This questionnaire consists of 19 questions and assesses the domains of desire, arousal, lubrication, orgasm, satisfaction and pain symptoms. We added one question to this, with the aim of quantifying the impact of urine loss during sexual intercourse.
On the eve of the surgical procedure, the patients received detailed explanations about the study and authorization to consent was obtained.
Firstly, the patients were asked objectively about their basic disease, with the aim of characterizing the urine losses that they presented. They were also asked about previous surgical procedures that they had undergone, with emphasis on procedures with vaginal access. Next, the modified FSFI was applied. The patients were requested to respond spontaneously to the questionnaire and take as much time as needed to answer it, in a calm environment without anyone else present. In the event that doubts arose with regard to any item, the investigator was called to provide the clarifications needed.
Six months after the surgical procedure, the patients who had had any sexual activity were invited to attend the urogynecology outpatient clinic at a prearranged time. First of all, they were asked about how successful the procedure had been with regard to urine losses and about any complications that may have occurred. Following this, the modified FSFI was again applied, under the same conditions as adopted previously.
For the continuous quantitative variables, the analysis described was done by means of observing the minimum and maximum values and calculating the means, standard deviations and medians. For the category variables, absolute and relative frequencies were calculated. When it was necessary to verify whether there were associations between the category variables, the chi-squared association test was utilized, or the Fisher exact test if at least one expected frequency was less than five.
To verify the impact of the surgical procedure performed, by means of comparisons between the mean scores for the items of desire, arousal, lubrication, orgasm, satisfaction, pain and urine losses, the non-parametric Wilcoxon signs test was applied.
The significance level utilized for the tests was 5%.
RESULTS
In accordance with the age distribution of the patients, they were subdivided into two groups. The first group included patients whose ages were less than or equal to 50 years and the second consisted of women whose ages were over 50 years (Table-1).
Among the 64 patients who underwent surgical treatment for SUI using the SST, 39 patients (60.94%) had had sexual activity during the preceding four weeks, while 25 patients (39.06%) had not had sexual activity. The distribution by age group for the patients with and without sexual activity is shown in Table-1. Application of the chi-squared test to these two samples gave p = 0.011; therefore, age group had an influence on sexual activity (significant value).
Among the patients without sexual activity, the reasons given for this abstinence were lack of partner; absence of sexual desire; partner with illness that impeded him from having sexual activity; and illness of the patient herself (Table-2). Only two (8%) of the 25 patients without sexual activity presented scores of six or over on the sexual desire question, while the range found was from 2 to 10.
Table-3 shows the means, standard deviations and medians for the 39 patients with sexual activity, for all the domains evaluated. Of these patients with sexual activity, 23 (59%) said they had urine losses during the sexual act, while 20 (51%) presented urine losses on 50% or more of the occasions when they had sexual activity (scores of 3, 2 and 1) and 13 (33%) had losses on all such occasions (score of 1).
Among the 39 patients who underwent treatment for SUI using SST, only one patient did not attend the outpatient clinic six months after the surgical procedure to answer the questionnaire again.
With regard to sexual desire, 10 patients (26.31%) presented increases in their scores, 24 remained unchanged and four had decreases. With regard to arousal, lubrication, orgasm, satisfaction and pain, similar distributions were found, i.e. more than 60% of the patients kept the same scores, while 10% to 30% of the patients had upward or downward changes in their scores. With regard to urinary loss during sexual activity, 20 patients (52.63%) presented an improvement, while 18 (47.37%) maintained their scores. No patient mentioned any worsening of the urine loss during sexual activity after the operation (Table-4).
Table-5 shows the mean preoperative and postoperative scores and significance levels after applying the non-parametric Wilcoxon signs test, which was utilized to compare the mean scores of the different domains of the FSFI. It could be seen that the items of arousal, lubrication, orgasm and totaling presented slight reductions, while desire, pain and urine losses had slight increases. However, none of these except for the domain of urine losses during the sexual act presented significance levels of less than 5%.
COMMENTS
At the beginning of 1999, the National Health and Social Life Survey (NHSLS) published a study in the Journal of the American Medical Association (2) that showed that sexual problems were more frequent than had been imagined; 43% of American women presented SD, a percentage that was higher than for men, whose rate was 31%. These data concur with the high incidence of women without sexual activity found in the present study, since 39.06% of the patients (29 women) were not having sexual relations. Despite the low number of women in the sample studied, the presence of women aged less than 50 years in this group and the fact that this was specifically a sample of women with SUI, it was observed that age had a significant influence on whether or not there was sexual activity. This result finds confirmation in the study by Diokno, with patients aged over 60 years: the latter study demonstrated that sexual activity diminishes with age and when the individual is single (5).
In the present study, patients explained their lack of sexual activity in different ways, but absence of desire was the principal factor that impeded sexual activity among these women, which can be demonstrated from the scoring for these patients. Thus, only two of these women (8%) had scores of greater than or equal to 7), while 21 (84%) presented scores of less than 4.
Out of the 20 patients without sexual activity that we managed to contact after a year of surgical procedure, 3 (15%) returned to sexual activity. In two cases, this sexual activity retaking was associated to the fact of not presenting urinary losses to the efforts, demonstrating that urinary losses can decrease sexual desire, considering that both patients related such losses as the cause for the low desire for sexual activity.
Sutherst & Brown evaluated the conjugal relations and sexual habits of 208 patients in a urinary incontinence clinic and observed that 43% of them said that urinary disorders had a negative effect on their sexual relations. According to these authors, women with vesicle instability had significantly greater incidence of SD than did women with genuine stress urinary incontinence (6).
A study by Iosif found that 20% of the women had diminished frequency of sexual activity and 5% were completely inactive sexually, as a result of urine losses during coitus or during the night (7).
In a prospective study, Haase & Skibsted reported that 35% of their patients had diminished libido in relation to the period prior to their urinary incontinence. Among such patients, 11 attributed the disorder to vaginal descensus or urine loss during the sexual act, and three just to the fear that such losses might occur. Twenty-three (42%) complained of dyspareunia (8).
In the present study, 23 women (59%) presented urine losses during sexual intercourse and, 20 (86.96%) of them said they had losses on at least half of such occasions, while 13 (56.72%) had urine loses on all occasions. Although this was not objectively asked about, it should be considered to be the preponderant factor in determining an unsatisfactory sexual life. The insecurity brought about by urine loss during day-to-day activities causes discontent and, with the intention of avoiding such losses, some activities are not done. Thus, what can be said about urine losses during intercourse? There is no doubt that it is a factor that must be taken into account when considering the low rates of desire, orgasm and satisfaction seen among preoperative patients. For this reason, when dealing with patients with UI, the negative impact that this has on female sexual life must not be forgotten. It is therefore appropriate to objectively probe patients about urine losses during the sexual act, as well as attempting to assess their impact.
Clark & Romm evaluated the effects of UI on sexual function by means of a questionnaire, and found that 56% of their patients had already experienced urine losses during sexual activity. These rates reached 66% when the patients were asked about incontinence or urinary urge or frequency during the relations. These data, which are very similar to the findings of the present study, demonstrate the negative impact that urinary incontinence may have on the quality of life of patients with SUI (9).
Despite the high incidence of FSD and SUI, there are few studies that have sought to correlate the two diseases. It was found by Hilliges et al. (10) in 1995 that the more distal portions of the vagina have more nerve components when compared to the proximal proportions. These findings were similar with regard to the anterior and posterior walls; and the areas that commonly undergo incision during procedures to treat urine losses are richly innervated. Taking these findings into consideration, it must be emphasized that it is imperative to study sexual function after such procedures have been performed, with the aim of assessing their effects on FSD.
Between the mean scores before and after the surgical procedure performed, it was observed that there were slight increases in the domains of desire and pain and the total of the scores, and a large increase in the item of urine loss during sexual intercourse, while for the variables of arousal, lubrication, orgasm and satisfaction there were slight decreases. Upon applying the statistical test, however, the significance level was not reached for any of them except for the variable of urine loss during sexual intercourse, which increased from 3.34 to 4.82, which was very close to the maximum score of 5 for this variable. This result was already expected, given that the "gold standard" treatment for urinary incontinence today is the surgery that was performed (11).
The diversity of the results is probably the result of non-standardization of the studies, given that it was only in 1998 that the American Foundation of Urologic Disease (AFUD) defined and classified female sexual dysfunction (12).
Analysis of the literature with regard to the treatment of SUI and patients' sexual life after the surgical procedure has shown greater uniformity of results, despite the low number of published studies. Iosif (7) and Haase & Skibsted (8) demonstrated an improvement in sexual activity and accredited the result to the greater satisfaction and self-esteem among their patients.
Recently, Walsh et al. (13) identified improvement in sexual function among their patients and stated that the solution for incontinence during intercourse was strongly associated with the improvement in sexual activity. Maaita et al. (14), in a similar study, concluded that this surgery would not have a negative effect on their patients' sexual lives.
The relationship between FSD and UI has reached such proportions today that Rogers et al. (15) have proposed that women with urinary incontinence or prolapse of pelvic organs behave differently from other populations studied previously from a sexual point of view. Thus, results obtained that would possibly be attributed to a series of factors such as age, for example, may be related to UI or genital prolapse. These authors even proposed a specific questionnaire for evaluating the impact of these diseases.
There is constant concern regarding the possibility of interfering in vaginal sensitivity, resulting from the fact that the principal site of innervation is the location for the incision in the surgical procedure for treating urinary incontinence using the suburethral support techniques. Nonetheless, the improvement in the patients' self-esteem produced by the absence of urine losses during the sexual act justifies the results, even if the improvements obtained in the patients of the present study were not significant. On the other hand, the worsening in sexual function found in some patients was related to complications in the procedures carried out, such as maintenance of urine losses, prolonged urine retention, urgency, etc. The SST for treating SUI is safe and does not worsen patients' sexual activity, but new studies should be conducted, using objective measurements of sexual function if possible, with the aim of confirming the results obtained.
This work will need to be taken to greater depth in order to obtain objective assessments of female sexual function, perhaps with quantification of the free nerve terminations in the anterior vaginal wall before and after the operation. This would be a way of assessing whether the SST could cause damage to female sexual function, with the attempt to identify, which patients might present worsening of this function after the operation, and whether the type of suburethral support utilized might interfere with the result obtained.
CONCLUSIONS
Analysis of the results regarding the sexual function of the women with SUI who underwent surgical treatment using SST allowed the following conclusions to be reached: age had an influence on sexual activity; urine loss during sexual activity was frequent among patients with SUI. The surgery presented cure rates of more than 90% for urine losses during the sexual act and did not jeopardize the patients' sexual activity; the improvement in sexual function, when it occurred, was subjectively related to increased sexual desire and not the reduction in urine losses during sexual relations after the surgical treatment, and the patients cured of SUI did not present improvement in relation to sexual function.

Traduzione - Italiano EFFETTI A BREVE TERMINE SULLA FUNZIONE SESSUALE IN SEGUITO ALL’INTERVENTO CHIRURGICO PER L’INCONTINENZA URINARIA DA SFORZO TRAMITE L’USO DELLE TECNICHE DI SUPPORTO SUBURETRALE.
OBIETTIVI: Valutare l’effetto dell’intervento chirurgico per l’incontinenza urinaria nella funzione sessuale della donna e identificare il trattamento che potrebbe migliorare la funzione sessuale e soprattutto la qualità della vita.
METODI E MATERIALI: 64 donne eterosessuali con tali sintomi sono state studiate usando il questionario FSFI (indice della funzione sessuale femminile), modificato con l’introduzione di una domanda per valutare l’effetto della perdita di urina. Questo test è stato sottoposto prima dell’intervento chirurgico e dopo 6 mesi dello stesso.
RISULTATI: Fra queste 64 pazienti, il 60,94% ha una regolare attività sessuale, mentre il 39,06% non l’ha. Fra le pazienti sessualmente attive, il 59% di esse accusa perdite di urina durante i rapporti sessuali, e di queste, l’87% dichiara di avere perdite di urina nella metà o più dei rapporti.
Statisticamente non vi erano significanti differenze nella stima di desiderio, stimolo sessuale, lubrificazione, orgasmo, appagamento e dolore, e nell’insieme del risultato, tra il periodo antecedente l’intervento e 6 mesi dopo lo stesso.
Tuttavia, i risultati in merito alle perdite di urina durante l’attività sessuale migliorarono significatamene.
CONCLUSIONI: L’analisi dei risultati ha permesso di raggiungere le seguenti conclusioni: la perdita di urina durante l’attività sessuale era frequente tra le pazienti con incontinenza urinaria. La chirurgia di supporto suburetrale non compromette l’attività sessuale. Le pazienti curate dall’incontinenza urinaria da sforzo non dichiarano miglioramenti delle funzioni sessuali.

INTRODUZIONE
Studi epidemiologici hanno sostenuto che gli stessi processi patologici e i fattori di rischio associati alla disfunzione erettile maschile (età, ipertensione, fumo, ipercolesterolemia e precedente chirurgia pelvica) sono altresì associati alla disfunzione sessuale femminile (FSD).
La disfunzione sessuale (SD) è molto presente in entrambi i sessi, e varia dal 10% al 52% tra gli uomini e dal 25% al 63% tra le donne. I dati pervenuti dal MMAS (Studi sull’invecchiamento dell’uomo, Massachussetts) ha dimostrato che il 34,8% degli uomini di età compresa tra i 40 e i 70 anni ha una moderata disfunzione sessuale, il che è strettamente legato all’età e ai disturbi correlati (diabete, ipertensione e ipercolesterolemia) e stato emotivo. Si conoscono meno gli aspetti epidemiologici delle disfunzioni sessuali femminili (FSD).
L’incontinenza urinaria femminile (UI) è molto diffusa. Diokno (1995) ne riscontrò la presenza nel 10%-25% della popolazione nella fascia di età compresa fra i 15 e i 64 anni. Il numero aumenterebbe se si considerasse la popolazione più anziana. Vale la pena sottolineare che negli Stati Uniti il giro di affari esistente in merito alla UI ammonta a dieci miliardi di dollari l’anno.
Sebbene le SD e la UI abbiano una forte incidenza, sono pochi gli studi che hanno ricercato delle correlazioni tra le due patologie mirando a valutare gli effetti della UI sulla vita sessuale delle donne. Allo stesso modo ci sono stati brevi studi sul miglioramento che potrebbe essere ottenuto in seguito all’intervento chirurgico per la UI.
Considerando la forte presenza della SUI nell’ambiente, gli obiettivi del presente studio sono stati rivolti nel valutare: l’effetto della UI nella vita sessuale delle donne dopo l’intervento chirurgico tramite l’uso delle tecniche di supporto suburetrale (SST); se la risoluzione del problema potrebbe favorire il miglioramento nella vita sessuale delle pazienti, contribuendo quindi al miglioramento della qualità della loro vita.
METODI E MATERIALI
I casi di 64 donne eterosessuali dell’ambulatorio di uroginecologia sono stati studiati tra l’agosto 2001 e il settembre 2002. Tutte loro erano state indirizzate all’intervento della SUI mediante l’uso delle tecniche di supporto suburetrale (SST). L’età delle pazienti variava dai 33 agli 86 anni, con una media di 56 anni, uno scarto di 12.3 anni e un mediano di 55 anni.
Avevamo scelto di esaminare soggettivamente la funzione sessuale di tutte le pazienti in esame, per mezzo dell’uso della versione modificata del questionario FSFI (indice di funzione sessuale femminile) che fu concepito da: UMDNJ Robert Wood Johnson Medical School, Piscataway, NJ; University of Tennessee, Memphis, TN; University of Washington, Seattle, WA; University of Texas, Austin, TX; e Columbia University School of Medicine, New York, NY. Questo test prevede 19 domande e serve per misurare i livelli di desiderio, stimolo, lubrificazione, orgasmo, appagamento e sintomi di dolore. Abbiamo aggiunto una domanda, con lo scopo di quantificare l’effetto delle perdite di urina durante i rapporti sessuali.
All’inizio della procedura le pazienti ricevevano informazioni dettagliate in merito allo studio e si ottenevano quindi consensi e autorizzazioni.
Inizialmente alle pazienti veniva richiesto di descrivere oggettivamente il loro disturbo di base, con lo scopo di individuare le caratteristiche delle perdite di urina che presentavano. Veniva anche chiesto loro se avessero subito altri interventi chirurgici in passato, in particolare quelli che prevedevano l’accesso vaginale. In seguito veniva sottoposto loro il questionario FSFI. Si richiedeva loro di rispondere in modo spontaneo e tranquillo, usufruendo del tempo necessario, in un ambiente calmo e senza altre presenze. Nel caso in cui avessero avuto dubbi in merito alle singole domande, un responsabile avrebbe dato loro delucidazioni in merito.
Sei mesi dopo la procedura chirurgica, le pazienti che avevano avuto attività sessuale erano state invitate a frequentare il laboratorio di uroginecologia nel tempo prestabilito. Prima di tutto, si chiedevano loro informazioni sul successo dell’intervento, con particolare attenzione rivolta alle perdite di urina e ad altre complicazioni che potevano essersi verificate. Dopo di ciò, veniva sottoposto il questionario FSFI modificato, secondo le stesse condizioni adottate precedentemente.
A causa delle continue variazioni quantitative, l’analisi descritta fu eseguita per mezzo dell’osservazione dei minimi e dei massimi valori e calcolando i (valori) medi, lo scarto quadratico medio e le mediane. Per la categoria delle variabili, sono state calcolate variabili assolute e relative. Qualora fosse stato necessario verificare se vi erano associazioni tra le diverse categorie delle variabili, veniva utilizzato il test di associazione chi-quadro (variabile causale), oppure il Fisher Exact Test, se almeno una delle frequenze previste era minore di 5.
Per verificare l’effetto della procedura chirurgica compiuta, mediante la comparazione tra il punteggio medio per ambito di desiderio, stimolo, lubrificazione, appagamento, dolore e perdite di urina, fu sottoposto il Non-parametric Wilcoxon signs test.
Il livello significativo utilizzato per i test fu il 5%.
RISULTATI
In merito alla suddivisione dell’età delle pazienti, furono formati 2 gruppi. Il primo gruppo includeva pazienti di età uguale o inferiore a 50 anni, il secondo le pazienti di età superiore. (Tabella 1).

Tra le 64 pazienti che subirono l’intervento chirurgico per la SUI per mezzo delle SST, 39 di esse (60,94%) avevano avuto attività sessuale durante le 4 settimane precedenti, mentre 25 pazienti (39,06%) non avevano praticato. La suddivisione per gruppi di età per le pazienti con e senza attività sessuale è mostrata nella Tabella 1. L’applicazione del test del Chi-Quadro a questi due campioni diede p=0.011; perciò l’età di appartenenza ebbe influenza sull’attività sessuale (valore significativo).
Tra le pazienti senza attività sessuale, le ragioni date per giustificare questa astinenza furono mancanza di partner, assenza di desiderio sessuale, partner con malattie che impediva loro di avere rapporti sessuali, malattia della paziente stessa. (Tabella 2).
Solamente l’8% delle 25 pazienti senza attività sessuale presentò un risultato di 6 o più nella domanda sul desiderio sessuale, mentre il campo di variazione trovato era da 2 a 10.

Di queste pazienti con attività sessuale, 23 (il 59%) dissero di avere perdite di urina durante i rapporti sessuali, mentre 20 (51%) presentavano perdite di urina nel 50% o più delle occasioni in cui aveva rapporti sessuali (punteggi di 3,2 e 1) e 13 (33%) avevano perdite in tutte le occasioni.

Tra le 39 pazienti che subirono il trattamento per la SUI attraverso l’uso delle SST, solamente una paziente non si presentò in clinica 6 mesi dopo la procedura chirurgica per rispondere di nuovo al questionario.
Con particolare attenzione al desiderio sessuale, 10 pazienti (26,31%) ottennero un aumento del proprio punteggio, 24 rimasero invariati e 4 diminuirono. In merito a stimolo, lubrificazione, orgasmo, appagamento e dolore, ne risultò una distribuzione simile, infatti più del 60% delle pazienti ottennero gli stessi punteggi, mentre dal 10% al 30% delle pazienti ottenne aumenti o diminuzioni nei loro punteggi. Per quanto riguarda la perdita di urina durante l’attività sessuale, 20 pazienti (52,63%) presentarono un miglioramento, mentre 18 (47,37%) mantennero il loro punteggio. Nessun paziente menzionò peggioramenti delle perdite di urina durante i rapporti sessuali dopo l’operazione (Tabella 4).

Si potrebbe osservare che le variabili dello stimolo, lubrificazione, orgasmo e i totali presentavano una lieve riduzione, mentre il desiderio, il dolore e la perdita di urina un lieve aumento. Tuttavia, nessuna variabile (ad eccezione della perdita di urina durante l’atto sessuale) presentò livelli significativi al di sotto del 5%.

COMMENTI
All’inizio del 1999, l’Indagine sulla Salute Nazionale e la Vita Sociale (NHSLS) pubblicò uno studio nella Rivista dell’Associazione Medica Americana (2) che dimostrò che i problemi sessuali erano più frequenti di quanto si immaginava; il 43% delle donne americane presentava disfunzioni sessuali (SD), una percentuale più alta di quella maschile, che si aggirava intorno al 31%. Questi dati coincidono con l’alta incidenza di donne senza attività sessuale trovate nel presente studio, giacché il 39,06% delle pazienti (29) non stavano avendo relazioni sessuali.
Nonostante il basso numero di donne prese in esame nel presente studio, la presenza (in questo gruppo) di donne di età inferiore ai 50 anni e il fatto che questo era specificatamente un esempio di donne con SUI, è stato osservato che l’età ha un’incidenza rilevante, che vi sia o meno attività sessuale. Questo risultato trova conferma nello studio di Diokno, con pazienti di età superiore ai 60 anni: lo studio più recente dimostrava che l’attività sessuale diminuisce con l’età e quando l’individuo è single.
Nel presente studio, le pazienti spiegavano l’astinenza dall’attività sessuale per differenti motivi, ma era la mancanza di desiderio il principale motivo che impediva loro l’attività sessuale, e può essere dimostrato attraverso i risultati ottenuti da queste pazienti. Così solamente 2 di queste donne (8%) ottennero un punteggio maggiore o uguale a 7, mentre 21 (84%) ottennero un risultato minore di 4.
Tra le 20 pazienti senza attività sessuale che decidemmo di contattare dopo un anno dall’intervento chirurgico, 3 (15%) tornarono ad avere rapporti sessuali. In due casi la ripresa dell’attività sessuale fu associata al fatto che non sussistevano più perdite di urina sotto sforzo, in dimostrazione del fatto che l’incontinenza urinaria può originare diminuzione del desiderio sessuale, considerando che entrambe le pazienti attribuiscono a queste perdite la causa del basso desiderio di attività sessuale.
Sutherst & Brown esaminarono le relazioni coniugali e le abitudini sessuali di 208 pazienti presso una clinica per l’incontinenza urinaria e osservarono che il 43% di loro affermarono che i disordini urinari avevano un effetto negativo nella loro relazione sessuale. Secondo questi autori, le donne con instabilità della vescica hanno significativi miglioramenti della SD in confronto a donne con incontinenza urinaria genuina da stress.
Uno studio di Iosif provò che il 20% delle donne diminuirono la frequenza dell’attività sessuale e il 5% erano completamente inattive sessualmente, a causa delle perdit di urina durante il coito o durante la notte.
In un precedente studio, Haase & Skibsted riportarono che il 35% dei loro pazienti accusavano una diminuzione della libido in confronto al periodo precedente l’incontinenza urinaria. Tra questi pazienti, 11 attribuirono questo disordine al prolasso vaginale o alle perdite di urina durante l’atto sessuale, e solo 3 l’attribuirono al timore che questo potesse accadere. 23 pazienti (42%) lamentavano dispareunia.
Nel presente studio 23 donne (il 59%) presentavano perdite di urina durante i rapporti sessuali e 20 (86,96%) affermavano di avere perdite nella metà degli episodi di rapporti sessuali, mentre 13 (56,72%) accusavano perdite di urina durante tutti i rapporti sessuali. Sebbene non fu richiesto loro espressamente di discutere questo aspetto, dovrebbe essere tenuto in considerazione in qualità di fattore predominate nella determinazione dell’insoddisfazione nella vita sessuale.
L’insicurezza portava quindi a perdite di urina durante le attività di tutti i giorni causando disagio e, nell’intenzione di evitare queste perdite, alcune attività non venivano più praticate. Cosa si può affermare quindi in merito alle perdite di urina durante i rapporti? Non c’è dubbio che sia un fattore da tenere presente quando si considerano la bassa soglia di desiderio, orgasmo e appagamento nei pazienti prima dell’operazione. Per questa ragione quando ci si occupa di pazienti con la UI, l’impatto negativo che questo ha nei confronti della vita sessuale femminile non deve essere dimenticato. Perciò è consigliato analizzare oggettivamente le pazienti in merito alle perdite di urina durante l’atto sessuale, così come è importante accertarne le reazioni.
Clark & Romm analizzarono gli effetti della UI nelle funzioni sessuali attraverso l’uso di un questionario, e scoprirono che il 56% dei loro pazienti aveva già avuto esperienze di perdite di urina durante l’attività sessuale. Questa percentuale aumentò fino al 66% quando vennero considerati fattori quali incontinenza, stimolo urinario e frequenza – di perdite - durante le relazioni. Questi dati, molto simili a quelli individuati nel presente studio, dimostrano l’impatto negativo che l’incontinenza urinaria potrebbe avere sulla qualità della vita nei pazienti con la SUI.
Nonostante la forte incidenza della FSD e della SUI, esistono pochi studi che hanno tentato di correlare i due disturbi. Fu scoperto da Hilliges nel 1995 che più porzioni distali della vagina hanno più componenti nervose quando comparate a proporzioni prossimali. Questi risultati erano simili in relazione alle pareti anteriore e posteriore; e l’area che solitamente viene incisa durante l’intervento per trattare le perdite di urina è ricca di nervi. Considerato ciò, bisogna tenere presente che è d’obbligo studiare la funzione sessuale dopo che queste procedure sono state eseguite, con l’intento di accertare gli effetti della FSD.
Tra i valori medi prima e dopo l’intervento chirurgico è stato osservato che vi erano diversi lievi aumenti nei domini del desiderio e del dolore, e in generale in tutti i risultati, e un forte aumento nel dominio della perdita di urina durante l’attività sessuale, mentre per le variabili di stimolo, lubrificazione, orgasmo e appagamento vi erano lievi diminuzioni. In merito all’applicazione del test statistico, tuttavia, il livello significativo non fu raggiunto per alcuni domini, ad eccezione della variabile della perdita di urina durante i rapporti sessuali, che aumentò da 3.34 a 4.82, valore molto vicino al punteggio massimo di 5. Questo risultato era prevedibile, dato che oggi il trattamento “gold standard” per l’incontinenza urinaria è la chirurgia stessa compiuta.
La diversità dei risultati è probabilmente dovuta ad una “non standardizzazione” degli studi, visto che solamente nel 1998 la Fondazione Americana Disagi Urologici (AFUD) definì e classificò la disfunzione sessuale femminile.
L’analisi della letteratura, considerato il trattamento della SUI e la vita sessuale delle pazienti dopo la procedura chirurgica, ha dimostrato una buona uniformità dei risultati nonostante lo scarso numero di studi pubblicati. Iosif e Haase & Skibsted dimostrarono un miglioramento dell’attività sessuale e attribuirono il risultato al forte appagamento e alla stima di se stesse tra le pazienti.
Recentemente Walsh individuò miglioramenti nella funzione sessuale tra le sue pazienti e affermò che la soluzione per l’incontinenza durante i rapporti era fortemente associata all’aumento dell’attività sessuale. Maaita, in uno studio simile, concluse che questo intervento non avrebbe avuto effetti negativi nella vita sessuale dei suoi pazienti.
Oggi la relazione tra le FSD e la UI ha raggiunto proporzioni tali che Rogers ha suggerito che le donne con l’incontinenza urinaria o prolasso degli organi pelvici si comportano differentemente rispetto alla popolazione studiata precedentemente, da un punto di vista sessuale. Così, i risultati ottenuti che probabilmente sarebbero attribuiti ad una serie di fattori quali l’età per esempio, forse riguardavano la UI o il prolasso genitale. Questi autori proponevano inoltre un questionario specifico per valutare le reazioni di questi disagi.
Esiste una preoccupazione costante riguardante la possibilità di intaccare la sensibilità vaginale, essendo che il principale punto di innervazione è lo stesso punto che viene inciso durante l’intervento per il trattamento dell’incontinenza urinaria attraverso l’uso di tecniche di supporto suburetrali. Non di meno, l’aumento della stima di se stesse prodotto dall’assenza di perdite di urina durante i rapporti sessuali giustifica i risultati, anche se i miglioramenti ottenuti nelle pazienti prese in esame nel presente studio non sono significativi. D’altro canto, il peggioramento della funzione sessuale individuato in alcune pazienti era dovuto a complicazioni affiorate in seguito all’esecuzione della procedura, quali il mantenimento delle perdite di urina, ritenzione prolungata di urina, urgenza ecc. La SST per il trattamento della SUI è sicura e non danneggia l’attività sessuale delle pazienti, ma dovrebbero essere intrapresi nuovi studi che, oggettivamente, dovrebbero, se possibile, misurare la funzione sessuale con lo scopo di confermare i risultati ottenuti.
Questo lavoro necessiterà di un approfondimento al fine di ottenere chiarimenti sulla funzione sessuale femminile, magari attraverso la quantificazione delle libere terminazioni nervose della parete vaginale anteriore prima e dopo l’intervento. Questo potrebbe essere un modo per chiarire se le SST possono danneggiare la funzione sessuale femminile, con il tentativo di identificare quali pazienti presentino peggioramenti di questa funzione dopo l’operazione, e se la tipologia del supporto suburetrale utilizzata potrebbe interferire con i risultati ottenuti.
CONCLUSIONI
L’analisi dei risultati, riguardante la funzione sessuale delle donne affette da SUI che si sono sottoposte all’intervento chirurgico mediante l’utilizzo delle SST, permette di raggiungere le seguenti conclusioni: l’età influenza l’attività sessuale; le perdite di urina durante i rapporti sessuali erano frequenti tra le pazienti che soffrivano di SUI; la soluzione chirurgica descritta permette di asserire che in più del 90% dei casi la perdita di urina durante l’atto sessuale non compromette l’attività sessuale delle pazienti. I miglioramenti della funzione sessuale, quando avvengono, sono soggettivamente connessi all’aumento del desiderio sessuale e non alla riduzione delle perdite di urina durante i rapporti sessuali dopo l’operazione chirurgica; le pazienti guarite dalla SUI non presentano miglioramenti della loro funzione sessuale.
