Your added context changes things materially, I think. The computers in question are decommissioned or put out of service/operation (and can thus no longer be used/connected to the network), in my view. This is why something along the lines of "decommissioning", "disposal" or "out-of-service (status)" might be more appropriate than my previous suggestions.

Hence, "Aussonderungsmeldung" and "Aussonderungsformular" could be translated as "(asset) retirement notice/report" and "(asset) retirement form".
