I pretty much agree with your translation, I've just made a few adjustments. I'd give the same advice that I provided you with before - there should be no Chinese characters in an English translation. Even if it is 'just' a code, it should be an English one.

  -> I suggested to consider leaving the Chinese characters in there, and it depends what the translation is being used for. You could always translate the characters in a footnote.

neutral  Daniel Evans: I wasn't saying "do not translate them". I was saying you may consider leaving them in as it is a specific code/number that is used to identify the certificate. As I said a footnote might be a good solution.

  -> In my translation I have tried to be as complete and faithful to the souce text term as possible. Personal documents/certificates may be used as evidence in legal matters, this is why I disagreed with your suggestion of leaving characters untranslated.
