From what I understand, you don't want a translation but an explanation.

If it's your tax form, you probably had to fill out what your nationality is. To prove your claim (the nationality you are filling in), you have to say, passport number ###, birth certificate, or some document proving that nationality.

Interpreto que se trata de una persona con doble nacionalidad, a la cual, a la hora de hacer la declaración de renta, se le pregunta por cual de las dos nacionalidades quiere optar, la de su país de origen o la del país donde reside. El resto de las hipótesis, en mi concepto, carecen de sentido.
