In this type of situation, you can't go wrong with "användarvillkor" followed by whatever they apply to. If you Google "Användarvillkor för" with quotation marks, you'll see lots of examples of this.

The translation could be:
"Vid uppladdning av en video som en del av sin ansökan ombeds den sökande att godkänna användarvillkoren för uppladdning."

Even if it could be polished by someone who better know how and where this sentence will be presented.

The reason I flipped the sentence around, starting with "When uploading a video as a part of ...", is because I wanted to avoid the close repetition of "uppladdning" in a more direct translation, such as this:

"Den sökande ombeds godkänna användarvillkoren för uppladdning vid uppladdning av en video som en del av sin ansökan."

We could find another word for uploading, but why overcomplicate things?
