Yes, the additional 'l' is to indicate plural, lymph nodes. But I'm afraid it doesn't make it any clearer what they mean by 'delad'.

There is another sentence that may/may not make it clearer?

"Idag mastekomi + axillutrymning inklusive lgll infraklav + prim rekons hö bröst".
I assume "lgll" is another abbreviation for lymph node.

The one thing I'm sure of is that lgl doesn't stand for any granular lymphoma but for is one of many different abbreviations for "lymph node" (based on "lymphoglandula").

The 2A and 2B classes are the node classes according to the TNM system. However, I can't make any real sense of the "delad" part. I've thought of "sectioned", but that doesn't match the N2B class in any way. Therefore, my suggestion would be to translate it as "divided", mainly because I beleive that would be the most neutral way to render "delad" when not knowing what they want to tell.
