Summary of implantable cardioverter-defibrillator procedure
Hi all,
This is for US English, FWIW.
The patient has dilated cardiomyopathy and a left bundle branch block.
They were fitted with an implantable cardioverter-defibrillator and a cardiac resynchronization therapy device. However, the left ventricular electrode of the latter was noted to have displaced into the pulmonary vein. So the procedure is an attempt to correct this problem.
I'm not sure exactly what is being done and why (except to remove the misplaced electrode), how should I translate "a través" and "avanza"?
"Se desconecta el electrodo de ventriculo izquierdo del dispositivo ****a través del cual se avanza una guía metálica**** y se retira el electrodo desplazado completamente. Sobre la guía se avanza un introductor al interior de la vía venosa axilar y a continuación el catéter guia con el cateter tetrapolar para cateterización del seno coronario."
It later mentions the insertion (I guess to replace the one that's withdrawn) of "un electrodo de estimulación-sensado cuadripolar" and "Se comprueba mediante escopia correcto posicionamiento de ***electrodo en vena lateral***".

This type of procedure is not an open-heart surgery.
We reach the heart with minimal incision near the arm pit and using a guidewire that goes from a large vein (near the arm pit) all the way up to the heart - or in this case, all the way up to the device implanted in this patient's chest (and all the equipment removed or inserted is done so through this guidewire).
Considering that the Spanish and English syntax are very different I believe it's easier to start in the middle of the sentence and work your way to the beginning.
So I sugest this translation:
"By using a metal guidewire, inserted through device's left ventricle, it's possible to disconnect the misplaced electrode and remove it completely."

But you know what suits the context best. Surely the term you choose will also be a good option!

Esto es lo que creo que dice el original:
El cable del electrodo correspondiente al ventrículo izquierdo (dislocado en este caso) se desconecta del dispositivo y a través de dicho cable se hace avanzar una guía metálica
En cuanto a la duda de Anne, los electrodos sobre ventrículo izquierdo en el caso de la terapia de resincronización son epicárdicos y se ubican a través de accesos venosos que alcanzan la aurícula derecha y de ahí el seno coronario etc. Ver aquí por ejemplo;
La técnica de elección para estimular el ventrículo izquierdo en la resincronización ventricular es la inserción de un electrodo a través del seno coronario, haciéndolo progresar hasta implantarlo en una vena epicárdica del ventrículo

Sin embargo tú hablas de un desplazamiento de un electrodo intraventricular, no epicárdico, dado que el desplazamiento del mismo se produce desde el ventrículo izquierdo hacia las venas pulmonares. Dado que se alude a un acceso venoso al mismo, probablemente se trate de un caso de transposición de grandes vasos...

As for the "a traves" I am not really sure what it is going through here. Gramatically seems to me like it means it is passed through the device (could be), but maybe it means through the left ventricle (?and up through LA and to PV to retreive the electrode).
