Source text - Spanish 1.- Presentación. ¿Qué es?
ERA es un test genético personalizado para diagnosticar el estado de receptividad del endometrio en la ventana de implantación de la mujer.
Esta herramienta diagnóstica molecular permite analizar los niveles de expresión de 238 genes relacionados con el estatus de receptividad endometrial. En un microarray personalizado con sondas para estos 238 genes se hibrida una muestra de ARN obtenido del tejido endometrial. Después de la hibridación se leen las intensidades de la señal y un predictor informático específico clasifica las muestras en función de su perfil de expresión como Receptivas o No receptivas.
El Array de Receptividad Endometrial (ERA®) ha sido diseñado, desarrollado y patentado por IVIOMICS (PCT/ES2009/000386).
2- Objetivo. ¿Para qué sirve?
El test ERA sirve para evaluar el estadio de un endometrio y determinar si presenta un perfil génico de receptividad o no en el momento de la realización de la biopsia.
El análisis permite determinar la ventana de implantación personalizada y establecer en función del resultado una transferencia de embriones personalizada (pET)
3.- Indicaciones. ¿Para quién y por qué?
Este test ha sido probado en pacientes que han tenido fallo de implantación con embriones de buena calidad morfológica (al menos 3 transferencias de embriones fallidas en mujeres jóvenes o dos en pacientes con 37 años o más). Este test está indicado para pacientes con útero aparentemente normal y grosor del endometrio normal (≥6 mm) en las cuales estos no parecen ser el problema.
En aproximadamente un 20% de estas pacientes se detecta la ventana de implantación desplazada.
El análisis permite determinar la ventana de implantación personalizada y establecer en función del resultado una transferencia de embriones personalizada (pET)
4.- Ventajas. ¿Qué ventajas sobre otros test?
El test ERA ha demostrado una alta sensibilidad y especificidad en la detección de perfiles de expresión génica asociados a receptividad.
El método clásico de datación del estadio del endometrio se basa en criterios histológicos. Ese método se ha demostrado que no discrimina entre pacientes fértiles e infértiles y tiene un alto grado de subjetividad por lo que resulta en nula aplicación clínica.
5.- Muestra. ¿Cómo?
Para este análisis es necesario tomar una biopsia endometrial de la mujer en ciclo natural o en ciclo sustituido en un momento correspondiente a la fase receptiva del endometrio. Es decir, en día 21 del ciclo natural (LH+7 o 6 días después de la ruptura del folículo en seguimiento ecográfico de la ovulación) o tras cinco (5) días completos de impregnación de Progesterona en ciclo sustituido (HRT). Tras tomar la biopsia, debe ser introducida inmediatamente en un “Criotubo ERA” (suministrado por IVIOMICS) y que contiene un líquido que permite la preservación del tejido. El tubo con la biopsia se guarda durante unas horas en la nevera tras lo cual puede realizarse en envío a nuestras instalaciones a temperatura ambiente.
Una vez recepcionada la muestra se mantiene a -80ºC hasta su procesamiento.
6.- Envío. ¿Cómo, cuándo, dónde y quién?
La muestra se transporta en un tubo sellado y en sobre acolchado para amortiguar golpes. La muestra se enviará a temperatura ambiente con la compañía de mensajería de elección del cliente. El viaje a nuestras instalaciones, a temperatura ambiente, no debe superar las 72 – 96 horas.

Translation - English 1. What is the ERA?
The ERA is a personalized genetic test to diagnose the state of endometrial receptivity in the window of implantation in women. This molecular diagnostic tool is used to analyze the expression levels of 238 genes related to the status of endometrial receptivity. Probes for these 238 genes are hybridized with RNA obtained from an endometrial tissue sample in a custom microarray. After hybridization the signal intensities are read by a customized prediction computer and the samples are classified as ‘Receptive’ or ‘Non-Receptive’ according to their specific expression profiles. The Endometrial Receptivity Array (ERA ®) was designed, developed, and patented by IVIOMICS (PCT/ES2009/000386).
2. What is the ERA for?
The ERA test is used to assess endometrial status and to determine whether the patient’s endometrium had a ‘Receptive’ gene profile or not at the time of biopsy. The analysis reveals the timing of the implantation window and permits personalized embryo transfer (pET) timing based on the individual results obtained.
3. Uses. Who is it for and why?
This test has been tested in patients who have had implantation failure with embryos of good morphological quality (at least 3 failed embryo transfers for younger women or two in patients 37 years or more). This test is recommended for patients with apparently normal uteri, with normal endometrial thickness (≥ 6 mm), in which no problems are apparent. A displaced implantation window is detected in approximately 20% of these patients. The analysis allows the implantation window to be determined, allowing personalized embryo transfer (pET) to be performed based on these individual results.
4. What are advantages over the other tests?
The ERA test has shown high sensitivity and specificity in detecting gene expression profiles associated with receptivity. Endometrial stage dating has been classically determined based on histological criteria. However, this method has been shown not to discriminate between fertile and infertile patients and involves a high degree of subjectivity, meaning these results cannot be applied in the clinic. The ERA test overcomes these problems.
5. How are samples taken?
To perform this analysis it is necessary to take an endometrial biopsy from women in their natural or hormonally replaced cycles, at a time corresponding to the phase of endometrial receptivity. That is, in the natural cycle at day 21, seven (7) days after the surge in luteinizing hormone (LH+7, or 6 days after the follicle rupture, when this is monitored by ultrasound) or after five (5) full days of progesterone implantation in hormone replacement therapy (HRT) cycles. After the biopsy is taken it is immediately introduced into an ‘ERA cryotube’ (supplied by IVIOMICS) which contains a fluid that allows the preservation of the tissue. The biopsy tube is kept for a few hours in the refrigerator, after which it can be shipped to our facility at room temperature. After its reception, the sample is kept at -80 °C until it is processed.
6. Shipping. How, when, where, and who?
The sample is transported in a sealed tube, in a shock absorbing padded envelope, and is sent at room temperature by courier of the customer’s choosing. The postal journey to our facilities, at room temperature, should not exceed 72 to 96 hours. Source text - Spanish TITULO: ESTIMACION FARMACOECONOMIA DE LOS NUEVOS MEDICAMENTOS CONTRA EL CANCER
OBJETIVOS: La situación económica, ha demostrado que los recursos destinados al gasto sanitario son limitados, siendo necesaria una racionalización de su consumo. Los análisis farmacoeconómicos están cobrando una crucial relevancia. La Fundación ECO, con el objetivo de promover la calidad de la asistencia Oncológica plantea un análisis del grado de consenso en relación a la incorporación de las nuevas dianas terapéuticas y la integración de la farmacoeconomía a la hora de evaluar su eficacia.
METODO: Estudio realizado durante el 1ª Seminario ECO en 2010, mediante un Método Delphi Modificado. Un grupo de expertos analizó cuestiones sobre estimaciones farmacoeconómicas, analizando el grado de consenso obtenido. El proyecto se desarrolló en cuatro fases: 1) constitución del comité coordinador; 2) constitución del panel de expertos; 3) elaboración y envío de una propuesta de conclusiones del Seminario (encuesta por correo electrónico en dos rondas); y 4) análisis de resultados y elaboración de conclusiones consensuadas.
RESULTADOS: Participaron 35 expertos. En relación al coste anual tolerable para la aprobación de nuevos fármacos, el 68.8% consideró aceptable un coste por año de vida ganado entre 30.000-100.000€. El 9.3% encontró aceptable costes superiores a 150.000€. Con respecto al beneficio para la aprobación de nuevos fármacos, en neoplasias con una supervivencia media menor de 2 años, el valor mediano que se considero clínicamente relevante fue un aumento de 3 meses tanto en el tiempo a la progresión (TP), como en la supervivencia (SG). En las neoplasias con supervivencia media superior a los 2 años, se consideró clínicamente relevante un aumento de TP de 3-4 meses y de 4-6 meses en SG. De forma consensuada, se consideró que un tratamiento quimioterápico estándar es eficaz si produce una mejora en la mediana de supervivencia al menos, 3 o 4 meses a un mínimo del 20% de los pacientes tratados.
CONCLUSIONES: Los costes de los nuevos fármacos, son más elevados que los tratamientos tradicionales, siendo prioritario identificar subgrupos de pacientes con perfiles moleculares específicos candidatos para tratamientos dirigidos con una mayor eficacia. La asignación de los recursos disponibles a las intervenciones más eficientes, para obtener mejores resultados clínicos con menores costes y con mejor perfil subjetivo posible, permite racionalizar los gastos. Los estudios farmacoeconómicos son una herramienta básica para obtener unos mejores resultados sobre la salud acordes con los recursos disponibles y las demás necesidades de la población.
Translation - English TITLE: PHARMACOECONOMIC ESTIMATION OF NEW ANTICANCER DRUGS
OBJECTIVES: The economic situation has shown that health spending resources are limited, requiring rationalisation of consumption, in which pharmacoeconomic analyses are becoming of crucial relevance. The ECO Foundation, with the goal of advancing the quality of cancer care set out to analyse the degree of consensus on the inclusion of new therapeutic targets and the integration of pharmacoeconomics when evaluating their effectiveness.
METHODS: A study was conducted during the 1st ECO Seminar in 2010, using a modified Delphi method. A group of experts analysed questions about pharmacoeconomic estimates, and the degree of consensus achieved was analysed. The project was developed in four stages: 1) formation of the coordinating committee, 2) establishment of the expert panel, 3) preparation and submission of the proposed Seminar conclusions (via an email survey in two rounds), and 4) results analysis and drawing of agreed conclusions.
RESULTS: 35 experts participated. Regarding the annual tolerable cost for the approval of new drugs, 68.8% considered a cost per life year gained between €30000-100000 acceptable, and 9.3% found costs above €150,000 acceptable. With respect to the benefits of approving new drugs, in tumours with a median survival rate of less than two years, the median value considered clinically relevant was an increase of three months both in time to progression (TP) as well as overall survival (OS). In cancers with a median survival rate of over 2 years, a TP increased by 3-4 months, and 4-6 months for OS, was considered clinically relevant. By consensus, a standard chemotherapy treatment was considered effective if there is an improvement in the median survival rate of at least 3 or 4 months in 20% of treated patients.
CONCLUSIONS: The costs of new drugs are higher than traditional treatments; therefore the identification of patient subgroups with specific molecular profiles as candidates for targeted therapies with greater efficacy is a priority. The allocation of available resources to more effective interventions in order to obtain better clinical outcomes, at a lower cost and with the best subjective profile possible, allows expenditure to be rationalised. Pharmacoeconomic studies are a basic tool to obtain better health outcomes commensurate with both the available resources and the other needs of the population.

Introducción
Paciente con 63 años de edad, antecedentes de tabaquismo e hipertensión arterial en tratamiento con enalapril. Ante un cuadro de hematuria monosintomática acude a urgencias, donde la analítica realizada mostró resultados dentro de los límites de la normalidad, con niveles de creatinina de 0,9 mg/dL y una tasa de filtrado glomerular (TFG) superior a 60 mL/min. La ecografía urológica realizada mostró un tumor en la pared posterolateral derecha de la vejiga de 4cm con aspecto infiltrante, hecho que llevó a remitir al paciente al Servicio de Urología. Allí, la Tomografía Axial Computarizada (TAC) del área abdómino-pélvica mostró una tumoración a nivel vesical con aparente infiltración de grasa y sin afectación ganglionar en estadio cT3b. Se realizaron cistoscopia y resección transuretral (RTU) del tumor. El tacto bimanual mostró una masa móvil no adherida al recto detectable antes y después de la resección. El Servicio de Anatomía Patológica concluyó que se trataba de un carcinoma urotelial con infiltración de la capa muscular y base afecta, persistiendo el tumor tras la RTU.
El paciente no recibió quimioterapia neoadyuvante, planificándose tratamiento con cirugía, que consistió en cistectomía radical y reconstrucción de Bricker-Wallace. Anatomía Patológica describió carcinoma urotelial infiltrante de grado 3 con infiltración de grasa. Veintidós adenopatías libres de neoplasia fueron extirpadas. El tumor se estadificó como pT3bN0. Los márgenes se encontraban libres de patología, del mismo modo que los uréteres. No había presencia de carcinoma in situ. Tras la cirugía, los niveles de creatinina eran de 1,2 mg/mL y la TFG era superior a 60 mL/min.
El paciente permaneció libre de enfermedad hasta 18 meses más tarde, cuando se evidenció un nódulo pulmonar único de 1,6cm de nueva aparición y no biopsiable. No se observaron ni adenopatías mediastínicas ni evidencia de enfermedad en ninguna otra localización.
Finalmente se realizó exéresis por parte del Servicio de Cirugía Torácica y los resultados de Anatomía Patológica fueron de metástasis por carcinoma urotelial de 1,2cm con bordes libres. No se realizó tratamiento con quimioterapia posterior.
A los 12 meses el paciente presentó recaída ganglionar múltiple retroperitoneal e ilíaca, la mayor de las cuales de 2cm de diámetro. Hidronefrosis II/IV derecha secundaria. La analítica se encontraba dentro de los límites de la normalidad a excepción de la creatinina (1,4 mg/dL) y la TFG, que era de 51 mL/min.
El paciente recibió dos ciclos de tratamiento con cisplatino/gemcitabina y, ante la disminución de la función renal, con niveles de creatinina de 1,6 mg/dL y una TFG de 45 mL/min se cambió el esquema a carboplatino (AUC 5) y gemcitabina, combinación de la que se administraron 4 ciclos.
Después del tercer ciclo de tratamiento el paciente presentó respuesta parcial muy importante, que se mantuvo tras 6 ciclos.
El paciente permaneció libre de progresión durante 8 meses, tras los cuales presentó nueva recaída pulmonar y ganglionar a nivel de mediastino.
Translation - English Controversies in bladder cancer
Introduction
Patient 63 years of age, with a history of smoking and hypertension, treated with enalapril. They visited the emergency department upon finding symptoms of a monosymptomatic haematuria, where the analysis performed showed results within normal limits, with creatinine levels of 0.9 mg/dL and a glomerular filtration rate (GFR) greater than 60 mL/min. Urological ultrasound showed a 4cm tumour on the right posterolateral wall of the bladder which appeared to be invasive, a fact that led to the patient’s referral to the urology department. There, X-ray computed tomography (x-ray CT) of the abdominal and pelvic area showed a stage cT3b mass at the vesical level with apparent infiltration of the fat but without lymph node involvement. Cystoscopy and transurethral resection (TUR) of the tumour was carried out. Bimanual palpation before and after resection showed a mobile mass which was not attached to the rectum. The Pathology Department concluded that it was a urothelial carcinoma with infiltration into the muscle layer, and based on this, that the tumour persisted after TUR.
The patient did not receive neoadjuvant chemotherapy, and surgical treatment was planned, which consisted of radical cystectomy and Bricker-Wallace reconstruction. The Pathology Department described a grade 3 metastatic urothelial carcinoma which was invading into adjacent fat. Twenty-two tumour-free lymph nodes were removed. The tumour was staged as a pT3bN0. The margins were pathology-free, as were the ureters. There was no in situ carcinoma. Following surgery, creatinine levels were 1.2 mg/mL and GFR was higher than 60 mL/min.
The patient remained disease-free until 18 months later when a single, newly formed and non-biopsiable 1.6 cm pulmonary nodule was discovered. No mediastinal lymphadenopathies were found and there was no evidence of disease in any other location.
The Department of Thoracic Surgery excised the mass and results from Anatomical Pathology showed that it was a 1.2 cm urothelial carcinoma metastasis with free edges. No chemotherapy was performed after the surgery.
At 12 months the patient presented multiple retroperitoneal and iliac lymph node relapses, the largest of which was 2cm in diameter. There was secondary II/IV hydronephrosis on the right side. Laboratory test results were within normal limits with the exception of creatinine (1.4 mg/dL) and GFR, which was 51 mL/min.
The patient received two cycles of treatment with cisplatin/gemcitabine, but with the decline of renal function, and with creatinine levels of 1.6 mg/dL and a GFR of 45 mL/min the scheme was changed to a carboplatin (AUC = 5) and gemcitabine combination which they received for four cycles.
After the third cycle of treatment the patient showed a very promising partial response, which was remained over six cycles.
The patient remained progression-free for eight months, after which a new lymph node and lung relapse was discovered at the mediastinal level.

Source text - Valencia En síntesi, a hores d’ara hom admet: 1) que l’ordinador quàntic serà capaç de rompre qualsevol criptosistema que hi faci ús repetit de la Clau de Xifrat, i 2) que només se li resistiran els criptosistemes de la classe on-time pad (com el de Vernam), de clau absolutament aleatòria i d’un sol ús. Ara bé, una clau de xifrat d’un sol ús és una conseqüència del fet que, fins ara, els dos interlocutors (Alice i Bob) hagin disposat només de criptosistemes que són vulnerables als atacs per força bruta. Però és ací on la tecnologia quàntica acudeix novament en la nostra ajuda: en un futur molt proper, la distribució de la clau de xifrat aleatòria es pot aconseguir a partir del Principi d’incertesa de Heisenberg i tot indica que en el context de la tecnologia actual no hi ha altra millor manera de fer-ho. En arribar ací, però, ens preguntem: són absolutament segures les afirmacions que hi acabem de fer? Resposta: al meu entendre, no. I anem a justificar aquesta apreciació, demostrant: 1) que l’ordinador quàntic no mai podrà rebentar determinats criptosistemas, que més endavant presentarem, tot i que la clau de xifrat s’usi repetidament, 2) que Alice i Bob poden transmetre’s la clau de xifrat aleatòria de manera absolutament segura sense necessitat de recórrer a la tecnologia quàntica, i 3) que actualment sabem i podem dissenyar criptosistemes més segurs que el de Vernam.
Translation - English In summary, we now have to admit: 1) that quantum computers are capable of breaking any cryptosystem that makes repeated use of encryption keys, and 2) that they can only resist on-time pad-class cryptosystems (such as that from Vernam), with completely random keys which are only used once. But, a single-use encryption key is the consequence of the fact that, so far, the two parties (Alice and Bob) only have cryptosystems that are vulnerable to brute force attacks at their disposal. But here quantum technology again comes to our aid: in the very near future, we will be able to obtain the distribution of the random encryption key using the Heisenberg uncertainty principle, and there is every indication that in the context of current technology there is no better way to acquire it. However, having arrived to this conclusion, we should ask ourselves: are we absolutely sure about the statements we have just made? The answer, in my opinion, is no. Will justify this view, by demonstrating: 1) that quantum computers can never break certain cryptosystems, and that, even so, the encryption key is used repeatedly; 2) that Alice and Bob can transmit the random encryption key absolutely securely without the need for quantum technology; 3) that we currently understand, and can design, cryptosystems which are more secure than Vernam’s.

La presencia de hiperandrogenismo leve durante la menopausia es un hecho frecuente, por el mantenimiento de síntesis de andrógenos de origen ovárico. Situaciones patológicas como el desarrollo de hirsutismo, siempre exige su estudio, siendo su diagnóstico difícil. Deberemos centrarnos en diferenciar su origen suprrarenal u ovárico, mediante una detallada anamnesis, hallazgos analíticos y en ocasiones técnicas de imagen. Presentamos en caso clínico, evolución y tratamiento de una paciente menopaúsica con un cuadro de virilización grave debido a una hipertecosis ovárica.

Summary
Mild hyperandrogenism frequently occurs during menopause because the ovaries continue to synthesise androgen. The appearance of pathological situations, such as the development of hirsutism, must always be investigated but can be difficult to diagnose. The starting point should always be to identify whether the source of excess androgen is of adrenal or ovarian origin by combining a detailed history with laboratory findings, and sometimes also imaging techniques. Here we present a clinical case of a menopausal patient who presented with severe virilisation due to ovarian hyperthecosis, and discuss the evolution of the case and its treatment.

• The Royal Spanish Federation for Athletics (RFEA) has published a list of the best marathons in Spain looking at both sporting and organisational criteria.
• The Valencian Marathon shared the podium with the the best half-marathon, the Valencian City Trinidad Alfonso Half-Marathon, which was also the best race in Spain, according to this ranking.
Valencia 28 May 2015. The Trinidad Alfonso Marathon in Valencia, organized by SD Correcaminos and the Valencian local government, boasts the first-ranking position among the best marathons in Spain according to the Royal Spanish Federation for Athletics (RFEA). This organisation prepares an annual ranking according to both organisational and sporting criteria for all the athletic events in Spain. The season runs from November to October and assigns a score annualy which establishes an average over the last three years which is ultimately used to create the final classification, placing the Valencian Marathon in first position this year.
The RFEA ranking put Valencia in front with 1,582 points, with a difference of more than 100 points over the second-placed Barcelona marathon, followed by the marathons in Sevilla, Madrid, and San Sebastian. A great achievement for a city that loves running, and which since 2011 has turned the trial around to make it a benchmark event at the international level, as well as an organisational and sporting example, as shown by the fact that since 2013 it has boasted the best scores in the country in the history of the rankings.
Paco Borao, president of SD Correcaminos, said he was “very proud to have the gold-standard trial in our country. It adds value to the whole team that has worked to make this possible, making the dreams of the volunteers and the whole club one more reason for the effort”.
The best half-marathon in Spain
In the same ranking by RFEA, the Valencian City Trinidad Alfonso half-marathon, organised by SD Correcaminos, established itself as the best race in its category by a large margin with respect to the other organised trials in the country. Valencia 28 de mayo de 2015. El Maratón Valencia Trinidad Alfonso, organizado por la SD Correcaminos y el Ayuntamiento de Valencia, se aúpa a la primera posición del ranking de los mejores maratones de España según la Real Federación Española de Atletismo (RFEA). Este estamento elabora anualmente un ranking atendiendo a criterios deportivos y organizativos de todos los eventos atléticos de nuestro país, cuya temporada se establece de noviembre a octubre. Asignada esta puntuación anualmente se establece una media entre los tres últimos años para, finalmente, elaborar la clasificación final que este año ha situado al Maratón Valencia como el número uno.
De esta forma, el ranking de la RFEA lo lidera Valencia con 1.582 puntos, con una diferencia superior a los 100 puntos del segundo clasificado, Maratón de Barcelona, al que siguen los maratones de Sevilla, Madrid y San Sebastián. Un gran mérito para una ciudad que se vuelca con el running y que desde 2011 le ha dado la vuelta a la prueba, convirtiéndolo en un evento de referencia a nivel internacional, así como un ejemplo organizativo y deportivo como evidencia el hecho de que desde 2013 ostenta la mejor marca de la historia en territorio nacional.
Paco Borao, presidente de la SD Correcaminos, se ha mostrado “muy orgulloso de poder contar con la prueba de referencia en nuestro país. Se pone en valor a todo el equipo que trabaja para que esto sea posible, convirtiendo la ilusión de los voluntarios y de todo un club en un motivo más para el esfuerzo”.

Además, a nivel global, se sitúa como la mejor carrera de España, con 2.139 puntos de media, obteniendo 2.779 en la pasada edición, merced a la organización de evento y al gran record conseguido en la edición anterior, con la sexta mejor marca de todos los tiempos.

At the Valencia Sport Expo, the Trinidad Alfonso Valencia marathon runner’s fair. This is being held in the Príncipe Felipe museum building in the Ciudad de las Artes y de las Ciencias, on the 13 and 14 of November 2015. It is open between 10am and 8pm. More information here.
The Trinidad Alfonso Valencia marathon and 10k will start at 9am on Sunday 15 of November 2015. Below is a general plan of the trial’s surroundings, to help to orientate you.
