Source text - English The use of oral antibiotics is one of the most common approaches in treating bacterial infections. Antibiotics can be delivered either systemically or by direct placement into the pocket around the dental implant. Each method of delivery has specific advantages and disadvantages. However, based on clinical and microbiological evidence, the type of microorganisms responsible for the infection is treated on a presumptive basis, founded on probabilistic reasoning. A wide range of antibiotic compounds and dose regiments is presented in the literature. Ideally, antibiotic treatment duration should include the shortest efficient cycle for preventing both clinical and microbiological relapse. However, this short cycle should ideally have certain characteristics such as rapid onset of action; bactericidal activity; lack of propensity to promote resistant mutants; ease of invasion into tissues; activity against nondividing bacteria; unaffected by adverse infection conditions (low pH, presence of pus, etc.); administration at an optimal dose; and an optimal and convenient dosing regimen.Local delivery facilitates the application of antimicrobial agents at levels that cannot be reached by the systemic route. However, these levels need to be maintained at a high local concentration for a long period of time, and the agents should reach the entire affected area, that is, the base of the pocket, in order to be efficient..
Translation - Ukrainian Застосування антибіотиків при встановленні дентальних імплантатів
Розповсюдженим методом лікування бактеріальних інфекцій є застосування пероральних антибіотиків. Застосовуються антибіотики широкого спектру дії або антибіотик вноситься безпосередньо в зубо-ясельну кишеню навколо дентального імплантата. Кожен метод має певні переваги та недоліки. Однак, зважаючи на клінічні та мікробіологічні підстави, лікування інфекції, викликаної певним типом мікроорганізмів є презумптивним і базується на вірогіднісній аргументації. Література містить широкий асортимент антибіотиків та схем їх застосування. Найкраще, якщо тривалість лікування антибіотиками складає найкоротший ефективний цикл для запобігання як клінічним, так і мікробіологічним рецидивам. Однак, необхідно, щоб характеристики цього короткого циклу включали в себе: швидкий початок дії, бактерицидну дію, відсутність здатності сприяти розмноженню стійких мутантів, полегшення від втручання в тканини, дію проти бактерій, що не діляться, незалежність від несприятливих умов інфекції (низький pH, наявність гною і т.ін.), введення оптимальної дози та оптимально зручна схема застосування. Застосування місцевого протимікробного препарату має переваги, яких не можна досягнути антибіотиком широкого спектру дії. Однак, для того, щоб місцевий антибіотик був ефективним необхідно підтримувати його високу концентрацію впродовж тривалого часу, а також, щоб діюча речовина покривала всю вражену зону, особливо дно зубо-ясельної кишені.
