Source text - English Bone mineral density at the lumbar spine and proximal femur was measured by dual-energy x-ray absorptiometry at base line and at six-month intervals (Hologic). All the scans were analyzed centrally.

English to Russian: Hypercalcemia
Source text - English Hypercalcemia is caused by an increase in (1) the mobilization of calcium from the bone, (2) intestinal calcium absorption, and (3) tubular reabsorption of calcium. Thus when the cause is in part due to an increase in intestinal calcium absorption such as patients with hyperparathyroidism, vitamin D intoxication, chronic granulomatous disorders, or lymphoma, patients can benefit by decreasing their dietary calcium intake to 600 to 800 mg a day and increasing their hydration. For patients with mild hypercalcemia (under 12 mg/dl) and no clinical manifestations, this often suffices until a cause is determined. Patients with a moderate elevation (12 to 14 mg/dl), with or without symptoms, usually benefit from treatment because the patients are often dehydrated, leading to a decrease in the GFR and the renal clearance of sodium and calcium. Patients with severe hypercalcemia (over 14 mg/dl) usually require immediate therapeutic intervention.

Source text - English Pre-operative hormonal therapy: In Study 024, similar incidence of AEs was reported for both treatment groups, hot flushes, headache, and nausea being the most commonly reported AEs.
Patients who underwent resection of their primary breast tumor following 4 months of neo-adjuvant treatment with LTR or tamoxifen were followed for 5 years (or until death if earlier). Post-surgery hormonal treatment, especially tamoxifen, was the most commonly prescribed adjuvant therapy. The median duration of adjuvant tamoxifen was 3.6 to 4 years. There were no significant differences between (randomized) treatment arms in event-free survival, loco-regional recurrence, distant metastasis, second primary cancers, or overall survival. Slightly under 40% of the patients in each treatment arm experienced at least 1 event; in about half of these patients, distant metastasis occurred. In total 77 deaths were reported, 28% in the randomized LTR arm and 19% in the randomized tamoxifen arm.
Of the 77 deaths, 42 were attributed to progression of the underlying breast cancer with a further 4 deaths attributed to second non-breast primary cancers.

To address the primary hypothesis, a total of 2292 patients will be enrolled in the study to yield approximately 2226 evaluable patients. It is anticipated that 1113 evaluable patients per regimen, assuming a 2-sided 5% significance level for testing the primary efficacy hypothesis and an expected response rate of 67.7% in each treatment regimen, would yield 90% power to declare non-inferiority for the single-dose FDM regimen, using a non-inferiority margin of 7 percentage points.
If the study design did not include a futility analysis, 1876 evaluable patients would be needed to have 90% power for the primary efficacy hypothesis. However, the inclusion of the futility analysis is accompanied by a slight loss of power. In order to maintain 90% power for the primary efficacy hypothesis, the sample size of the study was increased from 1876 to 2226 evaluable patients.

One of the most valuable experiences was at Practica, a Publishing House in Moscow, where I participated in translation of Harrison’s Principles of Internal Medicine and Goodman and Gilman’s Pharmacological Basis of Therapeutics.
